Hospice By The Bays AIDS Case Management Program provides a continuum of high quality, in-home skilled nursing, attendant care, case management, and psychosocial services to disabled persons with HIV/AIDS.
Persons with disabling HIV/AIDS may request services directly or be referred by a physician, community agency, or hospital discharge planner.
The AIDS Program Manager will arrange an intake visit. The client's personal physician will then be contacted to obtain certification of their diagnosis. During enrollment in the AIDS Program, each client, in collaboration with their personal physician, actively participates in developing a personal care plan.
Client satisfaction with program services is regularly monitored to ensure that a high level of quality is maintained.
